If I click the start quiz button in final assignment, should I just upload the PPT file (a lesson plan)? or else, Do I have to upload a lecture video, too?

No, you just upload your final assignment, preferably as a MSWord document or PDF. So, when you have completed it, go back to the final assignment page, click it, and submit/upload your final assignment.
