But in this context, we can know '친할머니' is Kim Nam-gyu's grandmother. That's why I interpreted her as 'his grandmother'.

-his 또는 her 이라고 번역되는 부분이 없기 때문에 이와 같은 문장에서는 I 또는 MY 라고 통역하면 됩니다. My grandmother....

네 주어진 문장만을 보았을 때 남규의(his) 할머니임을 알 수 없다는 말씀은 이해 했습니다.
하지만 my도 뭔가 부자연스러운 것 같아요. 문장을 읽어보면 자신의 이야기를 말하는 것 같지는 않아서요.
my라고 가정한다면 '가족들은' 이라고 하는게 부자연스럽지 않나요?
