Well, let's take a look at those questions:

The reason why it is true is that if students are filling in a chart, table, or graph while reading, it does help them to focus and create good notes.  Of course, this could also be done afterwards, but even though it is often referred to as a 'post-reading activity' it can also be done while students are reading and therefore it is 'true'. 

in regards to the difference between controlled and free practice activities, I can't recall right now at what exact moment I spoke about this topic.  Suffice to say that the difference between the two is similar to the difference between mechanical and meaningful forms in that free forms of practice allow students to use recently taught language in a "free" (which basically means independent) way, while controlled practice activities are far more limited.  Does that make sense?
